We are looking for a Senior Social Worker to join the Royal Borough of Windsor and Maidenhead (RBWM) Integrated Care Team. The Integrated Care Team is an innovative new team which combines NHS and Social Care services and is part of the NHS England move towards Integrated Care.

This includes Mental Health, Physical Health, Occupational Therapy and Social Work members. This multi-professional, multi-agency approach wraps care around the individual. Your role will draw on your expertise in order to react quickly and implement a holistic Social Care intervention plan to enable the service user to remain safe within their home environment.

We are co-located which means we have easy fluid communication with good access to all information systems. We provide innovative and planned solutions for anyone with joint health and social care needs. We have a daily huddle to work together on all cases, and a monthly Cluster where we are joined by GPs, Paramedics, Social Prescribers, and voluntary organisations to plan care for individuals with more complex needs. 

As the Integrated Care Team is multi-agency you will be professionally supported, and line managed by the Service Lead within the Optalis Physical Disability Team but will also receive support from your peers and manager within the Integrated Care Team.
