Please note this is not an RBWM role. RBWM are advertising this on behalf of Optalis.
This is a challenging and rewarding role within an established Short-Term Support and Rehabilitation team based within the community.
The team is vibrant, committed and works towards a common goal of promoting the independence of residents of the Royal Borough of Windsor and Maidenhead. The service assists to facilitate earlier discharge from hospital, prevent hospital admissions. The team comprises of occupational therapists, physiotherapists and rehabilitation assistants working in the community to support residents to achieve or regain independence. The service works closely with health partners to ensure the best outcomes for the service user.
As a social Care Practitioner, you will have a pivotal role in the dynamics of this fast paced dynamic person centred service.
In order to succeed you will be highly organised, with excellent interpersonal skills, and the ability to use your initiative. Experience in the Health or Social Care environment, particularly in care at home services is highly desirable, along with a full driving licence and daily use of a car.
