We are excited to offer an opportunity to join our Short-Term Support & Rehabilitation team as a Service Coordinator. This remarkable team comprises of Occupational Therapists, Physiotherapists and Rehabilitation Care Assistants working in the community to support residents to regain their independence. The team helps to facilitate earlier discharge from hospital, prevent hospital admissions, or support someone in the community who would otherwise be unable to remain at home safely in a crisis situation.
The Service Coordinator is a vital part of this team, carrying key responsibilities such as processing referrals into the service, supervising a team of Rehabilitation Care Assistants including allocating and coordinating their workload, carrying out initial assessment visits and undertaking a practical role to assist with the delivery of service when required. Due to the nature of this role, supervisory experience and an NVQ/CQF Level 3 in health and social care is desired. Furthermore, you are required to be highly organised with excellent interpersonal skills and the ability to use your initiative.
